1) Fill out a ‘consultation request’ form **here**.
After your submission is approved (allow 1-2 business days for processing), Dr. Ponce de Leon will contact you for a brief conversation. This free, 15 minute phone call will allow us to further determine if our services are a good fit for your needs.
If we decide to proceed with an initial evaluation, you will receive an invite to sign up for the patient portal.
2) On your patient portal, fill out all forms prior to your initial evaluation (i.e. consents, practice policies)
After signing these forms, you will be allowed to schedule your appointment for your initial evaluation.
A $200, non-refundable deposit is required to secure your appointment. This deposit will be credited towards the total appointment fee.
3) Attend your initial evaluation!
This initial evaluation is only a consultation.
After this intake appointment, you and your psychiatrist will decide whether or not to establish a doctor-patient relationship.
If you miss your first appointment, rescheduling the appointment will be at the discretion of the psychiatrist.

Initial consultation for Child/Adolescent (90 -120 min): $500
Initial consultation for Adult (90min): $400
Standard follow up/Medication Management only (30 min): $225
Extended follow up/ Medication management + Therapy/Therapy only (45-60 min): $325

An active card (credit/debit/HSA/FSA card) must be on file in order to schedule visits.
New patients will be billed a non-refundable deposit ($200) BEFORE the appointment, with the rest charged at the END of the appointment.
This deposit is non-refundable even if patient cancels or no-shows.
This deposit will be credited towards the total appointment fee.
Established patients will be billed automatically at the END of each appointment.

Cancellations made MORE than 24 hours in advance will NOT be charged.
Cancellations made LESS than 24 hours in advance will be charged HALF the appointment rate.
If patients arrive MORE than 10 minutes after their designated appointment time, they will be a considered a ‘no-show’ and charged the FULL appointment fee.

The majority of communication will occur via patient portal messaging, i.e. sending us a message for non-urgent matters. For more urgent matters, please leave a voicemail at the clinic number (626 559-2190).
All messages will be responded to within 1-2 business days.

If you, your child, or others are experiencing an emergency: Call 988/911 or go to the nearest emergency room.
Examples of emergencies include: the potential of any physical danger to you or others, or if you/your child are having thoughts of wanting to imminently hurt yourself or others.
